What is Personal Information and why do we collect it?

Personal Information is information or an opinion that identifies an individual. Examples of Personal Information I collect includes names, addresses, email addresses, phone and facsimile numbers. It also includes health information about patient/s who I consult with as part of Naturopathic consultation services.

This Personal Information is obtained in many ways including:

Email subscription through verbally agreeing for me to add your details, or entering your details through this website, or link through other sites.

Naturopathic consultations intake forms, including any information collected and provided by you via a patient management system such as HealthBank, or via my website www.kinshipwithnature.com.au

Through correspondence via social media such Facebook messenger, Instagram, Pinterest or other social media site. Telephone calls, interviews and email correspondence. Via links for course or webinar signups. From other publicly available sources, from cookies and third parties, such as but not limited to laboratory testing services. I don’t guarantee website links or policy of authorised third parties. We collect your Personal Information for the primary purpose of providing our services to you, providing information to our clients and marketing. We may also use your Personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. Personal information is used to set up patient ordering systems for patients who receive naturopathic consultations so they have access to order natural medicines accessible by prescription. You may unsubscribe from our mailing/marketing lists at any time by contacting us in writing. When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

Sensitive information is defined in the Privacy Act to include information or opinion about such things as an individual's racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record or health information. Sensitive information will be used by us only: • For the primary purpose for which it was obtained • For a secondary purpose that is directly related to the primary purpose • With your consent; or where required or authorised by law. Children You must be over 18 to access this site and consent to this privacy policy. If you are under 18, please exit this site immediately. By accessing this site, you agree that you are over the age of 18. We don’t knowingly obtain information from or about children under 18 on this site or market to them.

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

Your Personal Information may be disclosed in a number of circumstances including but not limited to the following: · Third parties where you consent to the use or disclosure of information pertaining to yourself or your child/dependant under your care or guardianship. This includes but is not limited to a supplier/s service to obtain natural medicine/s for your or your dependants prescription/s.

· You or your dependant/s doctor or other health care professional when you have given written or verbal permission for a minor under your care within a consultation for the purposes of providing appropriate care or written referrals.

· Non-identifiable information about you or your dependant/s for the purposes of mentoring and further professional education. ·        With an emergency contact, medical practitioner or 000 (emergency services) in the event of a serious threat to your or your dependant/s safety or life.

· Where required or authorised by law, such as but not limited to, a court order or subpoena.

· Analytical information collected by third parties, such as google analytics, Facebook, TikTok, Pinterest, other social media platforms or the Modern platform where www.kinshipwithnature.com.au is hosted.

Security of Personal Information

Your Personal or dependant/s Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorized access, modification or disclosure. For Naturopathic consultations I use the online Healthbank platform which is encrypted. They also have their own policies which you need to agree to for access. For any hard copy files, I store in a locked filing cabinet accessible only to myself. When your Personal or dependants Information is no longer needed for the purpose for which it was obtained we are required by law to store it in client files which will be kept by us for a minimum of 7 years.

Access to your Personal Information

You may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. Client notes from case taking during appointments are unable to be modified after 24 hours. If you wish a corrective note to be applied to the case file in regards to health information, it can be recorded as a new note in the next appointment. Contact details can be amended at any time. If you wish to access your Personal Information, please contact us in writing. Kinship with Nature will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information. In order to protect you or your dependant/s Personal Information we may require identification from you before releasing the requested information.

Maintaining the Quality of your Personal Information

It is an important to us that your Personal Information is up to date. We will take reasonable steps to make sure that your Personal Information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you.
